Intensive Transitions Team Case Manager Nursing-Transitions in Care 67962
The Intensive Transitions Team (ITT) Case Manager's primary responsibility is to facilitate patient care across the continuum for patients who are determined to meet the criteria for management by ITT. This is accomplished by managing, coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, utilization of services and financial goals. Oversees the discharge process for appropriate patients within the ITT population, in collaboration with members of the ITT. Collaborates with members of the ITT, hospital staff, primary care/specialty providers and post-acute partners to develop a patient-focused cross-setting plan of care. The Case Manager is integral to the team effort to establish a safe and appropriate discharge plan/plan of care. It is essential that the case manager possess a working knowledge of the spectrum of hospital and post-acute services that are available in the community.

Our medicine unit is unique in that we have single rooms with 3 negative pressure rooms that are used for patients requiring airborne isolation.
