Responsible for making home visits as needed, assisting in monitoring patient progress and assisting with discharge planning from the acute phase to the post-acute phase of the episode of care. Duties for Connect Care - will work collaboratively with team to ensure that referrals to appropriate community-based and post-acute partners are made, assist in connecting patient with primary care as needed, assess and secure DME, medications and other patient care items needed, and secure support services in collaboration with the team.
