Job Summary: The primary responsibility of this position is to provide supportive services to clients and their families enrolled in the SUNY Upstate Psychiatry High Risk Program (PHRP). The PHRP provides innovative recovery-based care for teens and young adults suffering from suicidal thoughts. The Case Manager is an important member of the PHRP care team, helping clients and their families with finances, housing, transportation, and coordination of care, therefore enabling many clients to attend and receive life-saving treatment that they might not otherwise of access to.
Minimum Qualifications:

Associate's degree with 1 year of related experience required.
Preferred Qualifications:

Bachelor' s degree, Qualified Health Professional (QHP), and previous case management experience working with individuals having mental illness is preferred.
Work Days:

Monday thru Fridays days
